Immunizations & Healthcare

As a college student, you need to start thinking about health care and hospitalization insurance.

Complete your immunization form
Did you graduate from a Minnesota high school in 1997 or after? If yes, you’re all set. If no, you can download a form from the Boynton Health Services Web site. Send your completed form to:

Boynton Health Services
410 Church Street SE
Minneapolis, MN 55455

Make sure you have hospitalization insurance
All students are required to have hospitalization insurance. Determine first if you will be covered by your own or your parents’ insurance plan. If so, bring this information with you to Orientation. You will be asked to provide the name of your insurance company and policy number at the time of registration. If you do not have your own insurance, will not be covered by your parents' insurance, or you fail to input hospitalization insurance information on the registration page, you will be automatically enrolled in the University-sponsored health benefit plan. To learn more about the University plan, visit www.bhs.umn.edu or call 612-624-0627.

Attention International Students
All international students are required to buy the Student Health Benefit Plan through Boynton Health Services.
